Tadalafil is a potent and selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5). By blocking PDE5, tadalafil improves blood flow into the penis, thereby facilitating the achievement and maintenance of an erection. Tadalafil has been tried for the treatment of pulmonary arterial hypertension (PAH) in randomized placebo-controlled trials (RCTs) and for the treatment of improving cardiovascular status in patients with PAH. However, the efficacy and safety of tadalafil for the treatment of pulmonary arterial hypertension (PAH) have not been established in the UK. In this placebo-controlled clinical trial, tadalafil 40mg was administered as an Erectile Dysfunction (ED) medication to adult men with PAH. Efficacy, safety and tolerability of tadalafil were assessed using a 24-week, double-blind, placebo-controlled study. Efficacy and tolerability were also assessed.

Introduction:The purpose of this study was to examine the effects of Tadalafil on the absorption and elimination of Tadalafil (brand name: Cialis). Tadalafil, the generic name for Cialis, is a phosphodiesterase type 5 inhibitor, which is used to treat erectile dysfunction (ED) and pulmonary arterial hypertension (PAH). The pharmacokinetics, pharmacodynamics, and therapeutic response of Cialis and Tadalafil in patients with ED and PAH were studied.
Methods:A total of 90 patients with ED and PAH (according to the American Erectile Dysfunction Association (EEA) criteria) and 60 patients with ED and PAH alone (according to the American PAH Society (APS) criteria) were enrolled in this study. Patients were administered the usual ED drug, and the treatment and control groups were compared in terms of demographic and clinical characteristics. Blood samples were obtained before and after Tadalafil administration. The Cialis and Tadalafil doses were determined according to the dosage of the active ingredient. The pharmacokinetic parameters were calculated and the bioequivalence of the drug to Cialis was assessed.
Results:In the Cialis dose-related pharmacokinetic parameters analysis, Tadalafil pharmacokinetic parameters showed a good bioavailability and a fast elimination. Moreover, there were no significant changes in Cialis clearance and plasma concentrations after Tadalafil administration. No significant changes were detected in the pharmacokinetic parameters after Tadalafil administration. The pharmacokinetic parameters of the Tadalafil dose-related pharmacokinetic parameters were significantly affected by the Cialis dose. Tadalafil administration increased the tmax (24.9 min) and AUC (0-24 h) and reduced the elimination (18.7%) and elimination half-life (23.0 min). The Cialis and Tadalafil pharmacokinetic parameters in the presence of the Cialis dose were also significantly affected by the Cialis dose.
Conclusions:In this study, the Tadalafil dose did not affect the pharmacokinetic parameters and bioequivalence of the Cialis and Tadalafil dose-related pharmacokinetic parameters. Tadalafil should be administered with caution to patients with ED and PAH who are taking drugs for erectile dysfunction, PAH, and ED. Therefore, Tadalafil is not recommended to patients with ED and PAH who are using drugs for ED, PAH, and ED.
